Current and Previous Patients: If you are currently or have ever been a patient of Stormont Vail Health or Cotton O’Neil Clinics you will be able to quickly request an activation code to setup a MyChart account. Fill out the Activation Code Request form below. It could take up to a week for processing and verification. Once approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. We have also developed a step-by-step video to guide you thru this process.

If you are a parent or guardian wanting access to your child’s account, please create your own account and then request access using the electronic request form found inside MyChart. For patients ages 14-17 year old, MyChart will display immunizations only. Talk to your provider about what options are available to you when it comes to accessing your adolescent's records.

Non-Stormont Vail Health Patients: If you are not currently or have never been a patient at Stormont Vail or Cotton O’Neil Clinics you will not be able to receive an activation code to create a MyChart account until you have created a patient record with us. To do this, you will need to call 785-270-0644 and our team will help you with this process.

If you are a parent or guardian wanting access to your child’s account, ages 0-13, please create your own account and then request access using the electronic request form found inside MyChart. For patients ages 14-17, MyChart is undergoing reconstruction. We will begin signing up new adolescents after the first of the year. Talk to your provider about what options are available to you when it comes to accessing your/your child’s records in the meantime.
